Department of Health's Immunisation web site and MMR the facts web site are government funded web sites maintained for your personal use and viewing. Access and use by you the user of this web site constitutes acceptance of our Terms and Conditions which take effect from the date of first use.

Except where information is specifically required when completing feedback or registration forms, this web site does not store or capture personal information but merely logs the user's IP address, which is automatically recognised by the web server.

1.3 Copyright licensing application forms

This applies if you want to reuse our material.

The Office of Public Sector Information (OPSI) collects personally identifiable information when you complete an application form for a licence to re-use Crown or Parliamentary copyright material, such as that published by Department of Health. OPSI will use this information to maintain a database of those companies and individuals who are licensed to re-use Crown or Parliamentary copyright materials.

1.4 How your information is used

We treat your e-mail address, telephone number and fax number as private information that might not ordinarily be known to others.

We do not object to you linking directly to the information that is hosted on our site. However, we do not permit our pages to be loaded into frames on your site. DH pages must load into the user's entire window.

We are not responsible for the content or reliability of the linked websites. Listing should not be taken as endorsement of any kind. We cannot guarantee that these links will work all of the time and we have no control over the availability of linked pages. Translations of immunisation information leaflets are provided on a case-by-case basis. In the majority of cases this means that most leaflets are translated into at least the Top 10 languages in most demand on the NHS Direct language line at the time of publication.

Summary of terms

Browser
